Listed at the bottom of this page are links to various forms.

 

Informational forms:

These forms provide important information about the camp including, check-in and check-out times, rules and regulations, directions, etc., and a Questionnaire which we would like completed and returned or brought to camp, if possible.

Medical and insurance information forms:

We request that you complete and return these forms so they will be available for our trainer and any medical personnel in the event that medical treatment is needed for your son.

We strongly suggest that the “Hoop Mountain Insurance and Medical History Information Form”  be on file with us. We prefer that they are mailed back to us, but they can alternatively be brought to camp and provided to us at check-in time. We do not require the forms to be completed. However, you are urged to return these important medical forms to us. In the case of a medical emergency, we do not want medical attention to be delayed because the camper’s medical history is not immediately available to medical staffIt is your responsibility to provide us with this information. If the forms are not received beforehand or brought to camp, campers /parents will be requested to provide some minimal emergency contact/medical information at check-in. 

If you plan on mailing the forms, please allow ample time for delivery by mail. We will not be in our Arlington Heights, IL, office after Friday, July 18th. In the event that you feel that there is not enough time for mail delivery, just have your son bring the completed forms to camp.

 

  • Medical and Insurance Information Form – In addition to providing the information requested, it is important that the form is signed by a parent on the bottom of page. Enclose a copy of the front and back of your insurance card.**
  • Release of Liability Form – Form should be signed by both camper and parent.
  • Hold Harmless In Lieu of Medical Insurance Form– This form is to be completed and signed if you do not carry medical insurance.

** Providing a copy of your insurance card is at your discretion. You may want us to have a copy of your insurance card on file to avoid any confusion should a medical situation occur. 

Please Note: Hoop Mountain Midwest carries Supplemental Insurance for each registered camper for his stay at camp.

 Fill out the downloaded form. When you click the "Submit by E-Mail" button, that will be your electronic signature and should send the form directly to us. We have provided electronic filing as a convenience to you, but have found that electronic filing does not always work as we would hope or as easily as it first appears. Since we want to have the forms on file at camp, if you have trouble, just download the forms and either mail or bring to camp. You can always call and ask whether or not your electronic submission has been received. We don't want you to think the forms have been received when they haven't.

You will need Adobe Reader in order to open and print these forms so they can be completed. Adobe Reader Version 8 or greater is required to Download and File Electronically.
